They’re often mentioned together and treated as interchangeable. However, they’re not the same medication. Understanding the differences between Ativan and Xanax can help you make sense of a prescription, recognize the anxiety symptoms being treated, and have a more informed conversation with your healthcare provider.

Xanax (alprazolam) and Ativan (lorazepam) both belong to a class of drugs called benzodiazepines (often shortened to “benzos”). This is why they function in almost the same way.
Both drugs work by enhancing the effect of gamma-aminobutyric acid (GABA), which is a neurotransmitter that slows down function in the central nervous system.[1] This results in a calming effect that reduces stress and anxiety symptoms, decreases muscle tension, and promotes sedation.
Because of this, benzodiazepines are effective anti-anxiety medications; however, they do pose a risk of developing physical dependence with extended use or at higher doses.[2] As a result, most doctors suggest using them for short-term treatment rather than an ongoing solution for anxiety.
Xanax is the brand name for alprazolam, a short-acting benzodiazepine commonly used for treating generalized anxiety disorder (GAD) and panic disorder.[3] Due to its rapid onset of action — typically within 15 to 30 minutes — it is frequently used by patients who require immediate relief from acute anxiety symptoms or panic attacks.
Its rapid onset is a contributing factor to Xanax carrying a somewhat higher risk of misuse than other benzodiazepines. Since it acts quickly and wears off relatively fast, some patients may find themselves wanting an additional dose sooner than their prescription calls for. Over time, this misuse contributes to tolerance and dependence.
Ativan is the brand name for lorazepam, an intermediate-acting benzodiazepine used for anxiety, seizure disorders, and short-term treatment of insomnia related to anxiety; it is also used off-label and in FDA-approved hospital settings for the treatment of alcohol withdrawal symptoms under medical supervision.[4]
Ativan typically takes longer to start working than Xanax does, but has a longer half-life, meaning that its calming effects tend to last longer between doses. Since lorazepam undergoes metabolic processing primarily through the liver, it is generally believed to be a safe option for people with liver disease or those taking multiple prescription medications, as it appears to produce fewer drug interactions than some other benzodiazepines.[5]

When taken at higher doses or combined with other drugs or substances such as alcohol, the side effects of these medications can intensify. Respiratory depression is a potentially fatal side effect of combining benzodiazepines with other CNS depressants such as opioids or alcohol.[6]
Combining benzodiazepines with opioids or antidepressants is the leading cause of benzodiazepine-related overdoses. Due to the dangers associated with this type of use, physicians are especially cautious when prescribing benzodiazepines to people taking opioids or antidepressants.
One of the most commonly asked questions regarding the comparison of Ativan and Xanax is if Ativan is “stronger” than Xanax.
The answer depends on how “stronger” is defined.
Milligram for milligram, alprazolam or Xanax is the more potent medication; a person would have to take a larger dose of lorazepam or Ativan to get the same results as a smaller dose of alprazolam. In this limited sense, alprazolam is the stronger medication.[7]
However, potency is not the same as overall effect or risks involved. Because of lorazepam’s longer half-life, the therapeutic effects are maintained consistently across the dosing interval.
Alprazolam, on the other hand, provides rapid onset and a relatively short duration of action, producing a greater fluctuation in how a person feels between doses. Some studies indicate that this variability may contribute to alprazolam having a slightly higher rate of misuse compared to lorazepam.[8]
There is no universal “better” medication.
The right choice for someone ultimately depends on that individual’s specific diagnosis, medical history, and how their body reacts to each medication. There are many people who prefer the fast-acting relief provided by alprazolam during panic attacks. However, there are also many others who do much better on lorazepam due to its steady effects, because they require medication for seizure disorders, or are undergoing acute alcohol withdrawal under medical care.
The decision about which of these two medications is the best fit for you should always be made with a licensed prescriber who knows your full history, not based on internet comparisons or someone else’s experience. Anxiety treatment also involves numerous alternatives to medication alone. Many practicing psychiatrists recommend using cognitive behavioral therapy (CBT) or other non-pharmacological treatments alongside short-term use of a benzodiazepine, especially for long-term management of anxiety and panic disorders.
Both lorazepam and alprazolam are Schedule IV controlled substances, meaning both medications have the capability of producing physical dependence with prolonged use, especially at higher doses.[9] Tolerance to benzodiazepines builds quickly, which drives people to increase dosage or frequency to achieve the desired result. This is one way dependence develops, often without the person being aware that dependency is forming until they attempt to stop.

Because withdrawal from benzodiazepines can be medically serious and even life-threatening in cases of long-term, high-dose use, stopping either medication abruptly is not recommended. A taper-down approach under physician supervision is the safest method to withdraw from a benzodiazepine. Medical detoxification is often indicated as the initial treatment for people with a significant physical dependence on a benzodiazepine.
